WHY THIS COURSE

The protocol works until a real patient sits down.

Most gender affirming care training teaches one protocol: start the hormone, order the labs, repeat every three months. Then you meet them.

The nonbinary patient

Wants voice changes but not facial hair. The protocol assumes a binary endpoint.

The trans man on testosterone

Worsening cyclic pelvic pain that everyone keeps calling atrophy.

Month nine

A hematocrit of 54. Cut the dose, or work it up first?

The 52-year-old trans woman

Hypertension, a smoking history, and a request for estrogen.

The protocol doesn't cover any of them. And "refer out" isn't an answer when there's nowhere to refer.

JOIN THE WAITLIST

Know everything, or do nothing.

The only field where those are the choices clinicians are given.

The Problem

You didn't get this in training. Maybe you got a lunch lecture on pronouns. So when a trans patient books, you're choosing between turning them away, a waitlist measured in months, or prescribing from a chart you found online and hoping nothing goes sideways.

And things go sideways. Erythrocytosis. Potassium rising on spironolactone. Unsatisfactory cytology. None of it is exotic medicine. It's primary care, in the version where the anatomy and the reference ranges both need translation.

This course is for you if

01

You've turned trans patients away or referred them out because "I'm not trained in that," and you're done doing that.

02

You can initiate hormones but freeze when the hematocrit climbs, the potassium rises, or the labs stop making sense.

03

You've stared at a lab report wondering which reference range applies to your patient.

04

Your patient's goals don't match the binary endpoints the protocols assume, and you don't know how to dose for "partial."

05

You can continue a maintenance regimen but aren't confident titrating over the course of a transition.

06

You want to do preventive care right for the anatomy in the room, not the anatomy the EHR assumes.

Not for you if

You want a plug-and-play protocol you can copy without thinking.

You're looking for one trick to fix every patient.

You treat adolescents. This course covers adult patients only.

CURRICULUM

Ten Modules, From the First Visit to the System Around It.

Module 1

Foundations and First Encounters

Terminology, epidemiology, minority stress, and models of care.

Module 2

The Initial Visit: Assessment and Informed Consent

Gender history without interrogation, goals-based planning, fertility counseling, and the baseline labs that get misread.

Module 3

Testosterone Therapy

Formulations, monitoring, and the full erythrocytosis playbook: when to act, when to watch, and what to work up before cutting the dose.

Module 4

Estrogen and Antiandrogen Therapy

VTE risk stratification, antiandrogen selection, and what the progesterone evidence supports and what it doesn't.

Module 5

Beyond the Binary: Individualized Regimens

Microdosing, partial-effect goals, cycling on and off therapy, and affirming care without hormones.

Module 6

Anatomy-Based Preventive Care

Organ inventory as the organizing principle: cervical, chest, prostate, bone, STI, and contraception care for the anatomy in the room.

Module 7

Pelvic Pain in Gender Affirming Care

Pelvic pain on testosterone is not automatically atrophy. Tucking-related pain, orchialgia, and endometriosis recognition.

Module 8

Surgical Care for the Non-Surgeon

Referral letters, perioperative hormone management, and the postop problems primary care sees first.

Module 9

Mental Health and the Whole Patient

Distinguishing dysphoria from co-occurring conditions, detransition handled clinically, and ruling out physiology before attributing symptoms to psychiatry.

Module 10

Systems, Documentation, and Practice Considerations

EHR workflows, lab reference ranges on hormone therapy, and gender-marker billing pitfalls.

CLINIC-READY TOOLS

What you can use on Monday.

Every tool is included with the course and built for a real visit.

Gender affirming SOAP note template

Compounded medications reference, evidence-graded, off-label status labeled

Documentation exercise with every module

Referral letters that meet WPATH SOC8 without gatekeeping

What Makes This Course Different

Built on real presentations

A hematocrit of 54, a potassium of 5.3, and a cyclic pelvic pain differential, worked through rather than listed.

Evidence-honest

Off-label is labeled off-label. Low evidence is labeled low evidence. The functional medicine content is held to the same standard as everything else.

Beyond the binary, literally

An entire module on microdosing, partial effects, cycling, and nonhormonal affirmation, because those patients are already in your waiting room.

Charted defensibly

Every module closes with a documentation exercise, because knowing it and charting it are different skills.

QUESTIONS

Before You Join.

No. The reasoning framework applies whether you prescribe, refer, or support. That said, Modules 3-5 are prescriber-depth pharmacology, so prescribers and those working toward prescribing will get the most from the full curriculum.

No. This course covers adult patients (18+) exclusively. Pediatric and adolescent gender care involves a distinct evidence base, consent framework, and regulatory landscape that this course does not address, although some lab interpretation and clinical concepts can overlap.

Modules 1 and 2 build the foundation: terminology, models of care, the initial visit, and informed consent. You don't need prior GAC experience. You do need to be willing to think, because this is a reasoning course, not a protocol library.

The parts the dosing charts skip: the erythrocytosis playbook, individualized and nonbinary regimens, anatomy-based prevention, pelvic pain workups, and perioperative management, nuanced patient counseling plus documentation practice in every module.

Module 7 delivers standalone competency in recognizing endometriosis in trans patients and referring appropriately. The full workup and management deep-dive lives in Filling in the Gaps™: Endometriosis & Chronic Pelvic Pain. You do not need that course to complete this one.

One year. Work at your own pace and revisit any module whenever you need it.

Accreditation has been submitted to Pinnacle Conference, LLC for joint providership (ACCME, ACPE, ANCC) and is pending. We expect 6+ contact hours, but credit is not guaranteed until it is confirmed. If you enroll before approval, you will be notified of the decision and can claim credit at completion once it is finalized.

At official launch and available to redeem upon course completion.
